About this role

Coordinates components related to the scheduling, admission, transfer, or consult of emergent, urgent, and elective patients into an accepting facility from referring hospitals, clinics, and physician offices. This role ensures appropriate level of care and timely, appropriate mode of transport.

Uses critical thinking skills to make recommendations that promote the best potential patient outcome based on clinical information, patient and physician preferences, and the capability of both transferring and receiving facilities. Screens admissions using approved criteria for appropriateness of level of care orders and bed assignments.

Requirements

Active Registered Nurse (RN) license required.
Intermediate proficiency in nursing patient assessment and evaluation.
Intermediate proficiency in care planning and resource coordination.
Intermediate proficiency in patient education.
Proficiency in electronic health record (EHR) data entry and office systems.
Expert-level clinical decision-making and critical thinking skills.
Ability to work night shifts (7 PM - 7 AM).

Responsibilities

Systematically process patient acuity and make expert level clinical decisions to facilitate patient flow through the continuum of care.
Use critical thinking skills to make recommendations that promote the best potential patient outcome based on clinical information, patient/physician preferences, and capability of both transferring and receiving facilities.
Screen admissions using approved criteria for appropriateness of level of care orders and bed assignments; coordinate patient scheduling needs for appointments and perioperative procedures.
Educate nurses, physicians, and ancillary staff on level of care and admission/transfer issues and opportunities.
Maintain proficiency in EHR data entry, telephone, and office systems to support operations while ensuring accurate, timely data entry and protection of PHI.
Stay up to date with all departmental procedures, policies, and communications to ensure adherence to appropriate process for final referral resolution.
Perform additional duties as assigned.
