About this role

This highly autonomous position involves taking the clinical lead across a portfolio of individuals, ensuring safe, effective, and person-centred care. You will work closely with support teams while providing clinical oversight, guidance, and leadership.

Daily work centres on undertaking holistic nursing assessments that cover physical, emotional, social, and wellbeing needs. You will develop and review person-centred care plans and complete clinical audits, reports, and documentation.

You will deliver clinical oversight and leadership to support staff teams while leading on best interest decisions and MCA processes. PBS4 is a values-led organisation that uses Positive Behaviour Support as the framework for services to people living in their own homes.

The organisation supports individuals with learning disabilities, autism, mental health needs, physical disabilities, and complex health needs across multiple counties. You will help raise standards of 21st Century clinical support in social care.

Requirements

Active NMC registration as a Registered Nurse (RGN or Learning Disability Nurse)
Strong clinical background in complex and long-term health conditions
Airway management, nutritional and enteral support, continence management
Neurological and medication skills, advanced monitoring, tissue viability
Experience in end-of-life care
Confident working independently with excellent critical thinking and decision-making skills
Excellent assessment, documentation, and report-writing skills
Driver with access to own vehicle and able to work across all shift patterns

Responsibilities
