In this full-time role, you will assess residents' health status and educational needs on admission and ongoing, communicating significant changes to the interdisciplinary team. You will develop and revise care plans based on identified needs, working closely with physicians, residents, families, and other team members.

Your day-to-day involves implementing nursing interventions such as medication administration, treatments, and IV fluids in a safe and timely manner. You will use critical thinking skills to handle changes in condition, notifying physicians via SBARR, and maintaining composure during crisis situations like family dissatisfaction or inclement weather.

You will monitor care delivery, identify problem areas using the QAPI process, and assist with call lights and resident care including ADLs. This role offers career advancement opportunities and the chance to stay current in best nursing practices through ongoing education and self-study.

Requirements

Education from an accredited school of nursing resulting in becoming a registered nurse
Current RN license in the state you will be working
CPR/AED certification required, or willing to obtain within six months of employment
Long-term care experience preferred
Must possess critical thinking skills
Excellent communication skills to interact verbally with patients, visitors, and staff
Excellent time management and organizational skills
Ability to stand and walk for long periods of time, lift heavy objects, and transfer/move patients weighing 200+ lbs using proper techniques

Responsibilities

Accurately assess residents' health status and educational/environmental needs on admission and ongoing
Communicate significant changes in condition to appropriate interdisciplinary team members
Develop and revise dynamic care plans using an interdisciplinary approach with physicians, residents, families, and team members
Identify and implement nursing interventions including medication administration, treatments, and IV fluid administrations in a safe and timely manner
Use critical thinking skills to address residents' change in condition and notify physicians using the SBARR communication tool
Maintain composure and handle crisis situations such as family dissatisfaction, staff disputes, or inclement weather in a calm and professional manner
Monitor care delivery to ensure the care plan is effective and use the QAPI process for immediate problem resolution
Assist with answering call lights and resident care including activities of daily living (ADLs)
