About this role
The Field Care Manager Nurse assesses and evaluates member needs to support health and wellness outcomes. You will partner with members, families, and care teams to develop and implement individualized care plans that coordinate services and connect members to resources addressing clinical and social needs.
This is a field-based and telephonic position where in-person versus telephonic engagement is guided by member needs, contractual requirements, and regulatory expectations. You will conduct comprehensive assessments to identify risks and barriers while developing member-centered care plans with measurable goals.
Field visits occur in members' homes, hospitals, skilled nursing facilities, group homes, homeless shelters, and correctional facilities as part of reentry planning. Telephonic outreach and follow-up support ongoing care management across providers and community support systems.
Evidence-based care pathways guide clinical decision-making while coaching and motivational interviewing techniques engage members. Monitoring progress and adjusting interventions ensures outcomes are achieved while addressing social determinants of health.
Requirements
- Active Registered Nurse (RN) license
- Bachelor's degree in nursing or healthcare/human services related field
- 2+ years of experience working as a case manager or care manager
- Experience working with adult populations
- Knowledge of community health services, social services, and available resources
- Proficiency in Microsoft Office (Word, Outlook, Excel) and electronic medical records
- Must reside in Western Kentucky, preferably in Caldwell, Lyon, Crittenden, Livingston, Hopkins, Christian, or Trigg counties
