About this role

The clinic Registered Nurse provides quality safe nursing care to patients of Siletz Community Health Clinic within the scope of practice. This role assists providers and other team members in a wide variety of clinical duties while coordinating care to optimize patient outcomes.

Participates in assessing, planning, and implementing patient care in a primary care setting. Performs direct care activities including administering medications, vaccines, or other treatments, then observes and documents patient response. Performs physical assessments, reviews histories, gathers data, and develops nursing care plans.

Communicates with personnel from other clinic departments, tribal departments, and community health partners to coordinate resources and resolve problems. Provides ongoing planning and monitoring of patient care. Communicates with patients via phone, face to face, or email to deliver test results, discuss medication changes, and provide education or instructions while promoting patient autonomy in health choices.

Identifies medically and psychosocially complex patients and coordinates referrals to health navigators. Utilizes motivational interviewing to encourage patients to set personal health goals. Actively participates in Quality Assessment and Improvement activities to support evidence-based best practices as outlined by Indian Health Service and AAAHC.

Requirements

  • Current RN licensure in the State of Oregon.
  • New graduate RNs with less than 2 years of experience will start at RN I (TL 124).
  • RNs with more than 2 years of experience will start at RN II (TL 125), with starting step negotiable.
  • Ability to work within scope of practice and comply with tribal, state, and federal regulations.
  • Knowledge of HIPAA guidelines and ability to maintain patient confidentiality.
  • Skilled in performing physical assessments, administering medications, and documenting patient care.
