About this role
Palm Medical Group is a not-for-profit, Federally Qualified Health Center (FQHC) focused on serving underserved populations with affordable primary care and preventative services. As an Advanced Practice Registered Nurse (APRN), you will provide direct care, health promotion, and disease prevention across the patient's lifespan within a family-centered practice.
Your day-to-day involves conducting patient exams, performing diagnostic tests, and developing treatment plans for acute and chronic conditions. You will educate and counsel patients on healthy lifestyle habits, prescribe medications within state and federal guidelines, and maintain thorough electronic health records. After-hours availability is required for triage and treatment coverage.
You will work in a patient-centered medical home (PCMH) environment that emphasizes continuity of care and strong patient relationships. Requirements
- Active Advanced Practice Registered Nurse (APRN) license in the state of Florida.
- National certification as a Family Nurse Practitioner (FNP) or relevant specialty.
- Current DEA registration for prescribing controlled substances.
- Master's degree in Nursing from an accredited program.
- BLS certification (American Heart Association).
- Experience in primary care or community health setting preferred.
Responsibilities
- Provide necessary medical services to patients within your field of training in accordance with PMG policies, procedures, and protocols.
- Educate, counsel, and guide patients on disease prevention and healthy lifestyle habits.
- Prescribe medication as defined by state and federal laws within your scope of practice.
- Develop treatment plans for acute and chronic diseases.
- Perform diagnostic tests, screening evaluations, and conduct patient exams.
- Be available for contact after hours to treat or triage medical conditions.
- Maintain the electronic health record (EHR) per current PMG policies and procedures.
- Participate in community service and health promotion programs.
