About this role
We are recruiting for a mission-driven Registered Nurse Care Coordinator (RNCC) to facilitate and coordinate care for an assigned patient group through multidisciplinary collaboration. This role ensures quality, cost-effective outcomes by assessing, planning, supporting, and evaluating patient care across diverse settings.
Day-to-day activities include conducting patient and family interviews, facilitating appropriate status and level of care, and progressing care with the healthcare team. Encounters occur face-to-face, telephonically, and electronically in clinic, home, school, and work environments to support seamless transitions.
This position is based in the Connections for Kids Clinic (CFKC), a medical home for children aged 0–18 in the custody of child welfare. CFKC offers trauma-informed, multidisciplinary care integrating medical, behavioral health, dental, and social work expertise with continuity across placement types and reunification.
The ideal candidate will contribute to developing care maps and clinical paths that improve patient population outcomes. The role also ensures compliance with state, federal, and third-party payor requirements while supporting a holistic approach to pediatric care.
Requirements
- Current RN license in the state of Colorado
- Bachelor of Science in Nursing (BSN) preferred
- Experience in pediatric nursing or community health
- Knowledge of care coordination and case management principles
- Ability to work effectively in a multidisciplinary team environment
- Familiarity with trauma-informed care and child welfare systems
- Competence in documenting and complying with regulatory and payor requirements
Responsibilities
- Use the nursing process to create patient- and family-centered goals and plans
- Collect and analyze assessment data to determine patient and family needs
- Develop evidence-based interventions and care plans supporting identified goals and outcomes
- Implement plans collaboratively with the patient, family, and care team
- Document care according to organizational, contractual, and regulatory standards
- Educate patients and caregivers on medications, treatments, and preventative care
- Incorporate cultural humility, language appropriateness, and health literacy into all patient interactions
- Coordinate care delivery within the clinic, across the organization, and between healthcare settings
