We seek an experienced and passionate Clinical Supervisor to assist in coordinating and managing home health clinical services. This full-time, in office-hybrid role involves daily supervision of nurses and home health aides to ensure quality within the plan of care. Responsibilities include reviewing clinical records and conducting necessary client and supervisory visits.

Our team empowers clinicians with support and resources to decrease administrative burdens, enabling optimal work-life balance. Work closely with the electronic medical record (EMR) to maintain compliance and provide training. Participate in staff appraisals, orientations, and professional development alongside the director of nursing.

Requirements

  • Active and Current Registered Nurse (RN) licensed in the state of New Jersey and have worked as an RN for at least three years
  • 1+ years of home health care or equivalent experience
  • Knowledge and experience in trach, vent, g-tube, and pediatric care
  • Ability to demonstrate self-confidence and positive attitude toward self and others
  • Ability to identify and evaluate personal strengths and weaknesses
  • Customer-service oriented
  • Working knowledge of, or proficiency in, Microsoft office applications, electronic medical records, and quick-to-learn proprietary software
  • Ability to communicate well, both verbally and in writing

Responsibilities

  • Provide daily supervision of nurses and home health aides to ensure quality of service within the required plan of care
  • Initiate and review clinical content, diagnoses, medical history, medications, and other statuses from client records
  • Appraise staff performance during probationary period and annually with the director of nursing, providing teaching and counseling
  • Identify and evaluate client needs and appropriateness of home care by reviewing initial intake screening forms
  • Assist with orientation of new professional staff to familiarize them with policies and procedures
  • Conduct client visits in the appropriate setting and initiate in-home supervisory visits of clinical staff as needed
  • Work closely with the electronic medical record (EMR) to ensure client charts are maintained, updated, and compliant
  • Conduct EMR chart audits and provide trainer sessions for peers and field staff
