About this role
Join Activa Home Health as a licensed Speech Language Pathologist in a Per Diem role offering flexibility, purpose, and meaningful work. Help patients regain speech, language, verbal independence, and confidence from the comfort of their homes. Deliver high quality, patient centered care in the community.
Provide hospital level skilled speech therapy services in patients’ homes, evaluating articulation, speech sound production, language comprehension, cognitive skills, social communication, oral motor skills, feeding, and swallowing. Develop and implement personalized treatment plans based on physician orders. Educate patients and caregivers to support therapy goals and communication strategies.
Accurately document visits and progress in the EMR system while monitoring outcomes and adjusting treatments as needed. Collaborate with an interdisciplinary care team for coordinated, high quality care. Comply with agency policies and state and federal regulations.

Requirements
- Active Florida SLP license in good standing
- Degree from an accredited Speech Therapy program
- Valid driver’s license & reliable transportation
- Current CPR certification
- Excellent communication, documentation, and time-management skills
- Level two background screening
- Recent negative TB test or chest x-ray
- Strong clinical knowledge in speech, language, pathology practice
Responsibilities
