Exalt Health Rehabilitation Conroe LLC is an acute rehabilitation hospital providing intensive rehabilitation programs. Patients must tolerate three hours of rehabilitation services daily while recovering from complex conditions including stroke, spinal cord injury, brain injury, and joint replacements.
The Registered Nurse delivers patient care through assessment, planning, evaluation, and implementation. This role directs patient teaching and guides ancillary personnel while upholding professional nursing standards in a fast-paced inpatient setting.
You will collaborate with physicians, therapists, and other healthcare professionals to support patients regaining functional skills. Daily work includes comprehensive assessments, medication administration, vital sign monitoring, and accurate documentation of progress and care.
The position emphasizes rehabilitation protocols, patient education on self-care strategies, and emotional support for patients and families. Strong clinical skills and a patient-centered approach help individuals work toward greater independence.
Requirements
- Current, unencumbered registered nurse license to practice professional nursing in the state where currently practicing.
- Current BLS via the American Heart Association or American Red Cross.
- Graduate of an accredited school of nursing.
- 1-2 years of medical-surgical nursing experience preferred.
- Strong clinical skills in assessment, medication administration, and patient care.
- Understanding of rehabilitation principles and a passion for helping patients regain their independence.
Responsibilities
- Conduct comprehensive assessments, administer medications, monitor vital signs, and provide skilled nursing care based on individualized patient plans.
- Assist patients with regaining functional skills through activities of daily living while following established rehabilitation protocols.
- Educate patients and families on the rehabilitation process, provide emotional support, and promote self-care strategies.
- Collaborate with physicians, therapists, social workers, and team members to ensure coordinated patient care.
- Accurately document patient assessments, progress notes, and medication administration.
